Medical Release Form Please fill out the following medical release form: Player Medical Information and Permission Form To download and fill out the form, click on the link above and select "Save As" in Internet Explorer or in Firefox. Once you have these filled out for each player, keep them on hand in case of a player injury and medical attention is needed.
